The State provides 24 hours paid leave in the event of death of an immediate family member. An additional 16 hours may be granted to attend a funeral out of state.

HEALTH INSURANCE

The State offers comprehensive health care coverage to eligible employees through a self-insurance program. The type of coverage for the current plan year is chosen by the employee at the time of employment. The State pays the major portion of the premiums for employees and dependents. Since the health insurance is part of a flexible benefits program, employee premiums are on a pre-tax basis.

DENTAL INSURANCE

Dental insurance is available to eligible employees and dependents, with premiums paid on a pre-tax basis. Employees may select dental insurance for themselves and their dependents through an indemnity plan or a pre-paid plan.

VISION CARE PLAN

The vision care plan enables eligible State employees to take advantage of group rates through payroll deductions on a pre-tax basis. This plan is voluntary and is fully employee paid.

RETIREMENT

The State has an excellent retirement program. An annually adjusted percentage of your gross wage is deducted on a pre-tax basis and matched by the State as contributions to your retirement fund. In addition, State employees are covered by Social Security benefits.

DEFERRED COMPENSATION

This optional program provides an opportunity to supplement retirement income by investing a portion of current income and deferring payment of federal and state income taxes until the money is received, usually after retirement.

LIFE AND DISABILITY

The State pays your basic life insurance. The basic life insurance is $15,000. Supplemental life (additional insurance) for you and your dependents is available at low group rates. Premiums for the first $40,000 of life insurance are taken on a pre-tax basis. Long Term Disability Insurance is automatically available to all eligible employees. If you become disabled, LTD Insurance will provide you with two-thirds of your base wages based on a specific formula after six months of continuous disability.

SHORT TERM DISABILITY

Short term disability insurance is available and is paid fully by the employee. It offers participating employees a salary continuation for a maximum of 26 weeks. Benefits (two-thirds of monthly salary) are payable on the 31st day for sickness, and on the first day for non-occupational accidents and for hospitalization. State employees injured on the job are covered by Workers' Compensation.

DEPENDENT CARE REIMBURSEMENT ACCOUNT

This Dependent Care Reimbursement Account (DCRA) allows an eligible State employee to deposit a portion of each paycheck on a pre-tax basis into a special account to pay for child care or care of a disabled spouse or elderly parent after submitting proper documentation to a third party administrator. DCRA is a flexible spending account, fully employee funded.

MEDICAL REIMBURSEMENT ACCOUNT

The Arizona Medical Reimbursement Account (AMRA) allows an eligible State employee to deposit a portion of each paycheck on a pre-tax basis into a special individual account to pay for out-of-pocket health care expenses for the employee and dependent (as defined by the Internal Revenue Service) after submitting proper documentation to a third party administrator. AMRA is a flexible spending account, fully employee funded.

ARIZONA HEALTHWAYS

The State Wellness Program, "Arizona Healthways," provides State employees a wide variety of health education classes, health screening, and intervention events, statewide. You can view more information about Arizona Healthways by checking out the Arizona Department of Administration website.

CREDIT UNION

State employees and dependents are immediately eligible to join a credit union. Credit unions provide many benefits at low costs, and can offer sound financial planning advice to you and your family. Depending upon the agency for which you work, you may become a member of DES Federal Credit Union, Grand Canyon State Employees Federal Credit Union, or Arizona State Employees Saving and Credit Union.
